Can carpal tunnel cause pain in my elbow or shoulder?

In short

Usually not. Carpal tunnel usually causes numbness, tingling and pain below the wrist, in the thumb, index and middle fingers. Occasionally the pain travels up toward the elbow or shoulder. Pain that sits mainly in the elbow or shoulder points to another cause, and Dr. Knight examines the whole arm to find it. He practices in Southlake and Plano, Texas.

Where carpal tunnel symptoms live

Let's take a look at where the nerve actually goes. The nerve that runs through the carpal tunnel, or what we call the median nerve, gives feeling to the thumb, index and middle fingers and part of the ring finger.1 That is where carpal tunnel symptoms normally sit, below the wrist. Dr. Knight covers the tunnel and the nerve in What is carpal tunnel syndrome?.

When it travels

Now and then the ache spreads up the forearm toward the shoulder, and that is a recognized symptom of carpal tunnel syndrome.1 It is the exception, though. When the main complaint is at the elbow or the shoulder, Dr. Knight examines the whole arm, because the same nerve symptoms can start at the elbow or in the neck. A nerve conduction study helps tell those apart.1

In Dr. Knight's words

"Typically carpal tunnel does not extend to the elbow or shoulder. Usually it is just numbness, tingling and pain below the wrist. But occasionally it can radiate up to the elbow or shoulder."

Dr. John T. Knight, recorded interview for The Hand and Wrist Institute, 2 November 2026.
