Nondiscrimination and language assistance
The Hand and Wrist Institute follows federal civil rights law. We do not exclude anyone, treat anyone differently or refuse care because of race, colour, national origin, sex, age or disability. If you need an interpreter, or you need information in a format you can read, we provide it free of charge. Call (817) 382-6789 and ask.
Notice of nondiscrimination
The Hand and Wrist Institute complies with applicable federal civil rights laws and does not discriminate on the basis of race, colour, national origin, sex, age or disability. We do not exclude people or treat them differently on any of those grounds.
This applies to everything we do: making an appointment, being seen, having surgery, being billed, and every communication in between.
What we provide, free of charge
For people whose first language is not English:
- Qualified interpreters, in person or by telephone.
- Written information in other languages.
For people with disabilities, so they can communicate with us effectively:
- Qualified sign language interpreters.
- Written information in other formats, including large print, audio, accessible electronic formats and other formats you ask for.
None of this costs you anything. Ask for it when you book, or at any point afterwards, by calling (817) 382-6789. Telling us in advance means we can have the right person with us on the day.
How to raise a concern with us
If you believe we have failed to provide any of the above, or that we discriminated against you on the basis of race, colour, national origin, sex, age or disability, you can file a grievance with us. Write to our Civil Rights Coordinator at The Hand and Wrist Institute, 510 E Southlake Blvd Ste 140, Southlake, TX 76092, or call (817) 382-6789.
Tell us what happened, when, and what you would like us to do about it. If you need help writing the grievance down, we will help you. We will not treat you differently for filing one.
How to file a complaint with the federal government
You can also file a civil rights complaint with the U.S. Department of Health and Human Services, Office for Civil Rights. You can do this instead of raising it with us, or as well as, and you do not need our permission.
- Online, through the Office for Civil Rights complaint portal at ocrportal.hhs.gov.
- By post, to U.S. Department of Health and Human Services, 200 Independence Avenue SW, Room 509F, HHH Building, Washington, DC 20201. Complaint forms are at hhs.gov.
