What does the appointment request form ask for?
The appointment request form asks for your name, email address, phone number, street address, preferred location, insurance carrier and the reason for your visit, so our team can contact you about scheduling and begin your chart before you arrive. It also asks, optionally, how you heard about us, and includes an optional SMS consent checkbox. Sending a request does not confirm an appointment.
The form does not ask for your insurance member ID or policy number, your date of birth, or a medical record number. If we need any of those our team will take them on the phone or at your visit.
Please do not include detailed medical information, test results or medical records. If the form cannot send your request, it displays an error with our phone number. Call (817) 382-6789 if you need help reaching our team.
What about online booking through Healow?
You can also follow the online booking link to Dr. Knight's profile on Healow. Booking information you enter there is submitted to Healow, not through a form on this website. Review the privacy information displayed by that service before entering personal information.
What should you do with medical records?
Call our team to ask how to provide medical records or other confidential information. If this is an emergency, call 911.
