Can De Quervain's heal on its own, and do I need a cast for it?
In short
Sometimes. De Quervain's can settle when you rest the thumb in a removable brace called a thumb spica, which crosses the wrist and runs to the tip of the thumb. Dr. Knight uses that removable brace, not a cast. In his experience, a steroid injection along with the brace for several weeks usually works best.

What is inflamed
Tuck your thumb into your palm, close your fingers over it and bend the wrist toward your little finger. That is a version of the Finkelstein test, and if it hurts along the thumb side of the wrist, it is pointing at the two tendons this is about.1 They run through a tight tunnel there, and moving the thumb, gripping and turning the wrist all rub them.1 That is why lifting a child can hurt, and why it is associated with pregnancy and the months after having a baby.1
Brace, then brace plus injection
The first step is to rest those tendons. Dr. Knight uses a thumb spica brace, which crosses the wrist and holds the thumb out to its tip. It is removable, so it is not a cast. On its own, the brace occasionally lets the tendons settle. In his experience, a steroid injection along with the brace for several weeks usually works best. If symptoms are severe or have not improved after six weeks of nonsurgical treatment, surgery to release the tunnel may be recommended.1 He explains the condition in What is De Quervain's tenosynovitis, or mommy thumb?.

In Dr. Knight's words
"Occasionally De Quervain's can heal by just wearing a removable brace called a thumb spica brace, that crosses the wrist and goes out to the tip of the thumb. Sometimes just doing that, it can heal, but usually the most effective is to do a steroid injection along with a brace for several weeks."
