What recovery after a carpal tunnel release actually means

Let's take a look at what is healing, because two separate things are healing at once and they do not finish together. One is the wound at the wrist crease, which is a skin and soft tissue problem and behaves like any other small wound. The other is the nerve that was being squeezed inside the tunnel, or what we call the median nerve, and a nerve that has been under pressure for a long time recovers on a schedule of its own once the pressure comes off.¹ When somebody tells you their recovery took a week and somebody else says it took months, they are usually answering two different questions.

The wound's clock, or the nerve's clock

So here is the distinction to hold on to, and it is the one that saves people a lot of worry. The wound is closed, dry and safe to shower over within days, and Dr. Knight dresses it for exactly that purpose. The nerve is a separate matter. Numbness, tingling and grip weakness follow how long and how hard the nerve was compressed before the release, not how the skin looks.

The test you can apply is simple. Look at the incision, then close your eyes and check your fingertips. If the incision has settled and the fingertips are still numb, those are two separate problems, and the second is not a sign the wound is failing. Report both at your follow up.

What is set at your visit, and why there is no table here

We are not printing a two number recovery range on this page. Dr. Knight states recovery figures on video for several other operations, but for endoscopic carpal tunnel release he does not put a back to normal activity number and a full healing number on the record, and we will not invent one for him. Here is what he does state, in order:

1. Ice and elevation start immediately after the operation to keep the swelling down.
2. The dressing is built so you can shower without waiting for the wound to be uncovered.
3. Motion of the hand and wrist starts early rather than after a long spell in a brace.
4. Severe cases behave differently, and he says so plainly rather than averaging them in.

Your own two numbers, the one for getting back to your normal day and the one for full healing, come out of the exam. Bring your job, your grip demands and how long the numbness has been going on, and ask for both figures before you leave.

When to call us

• Pain that is increasing after the first several days rather than easing.
• Fever, spreading redness around the incision, or drainage from the wound.
• A dressing that has come loose, soaked through, or got wet.
• Numbness that had lifted after surgery and has returned.
• Fingers on the operated hand that will not bend or straighten.
• Any new numbness or weakness in the hand that was not operated on.
