Does a broken wrist need surgery?

In short

Not always. A broken wrist whose pieces are still in line is usually treated in a cast, and surgery is for a break where the pieces have shifted or the joint surface has been disturbed. Dr. Knight makes that call from the x-rays, and sometimes from a CT scan, in the same visit.

What a broken wrist is

Run a fingertip down your forearm on the thumb side until you reach the crease of your wrist. The bone that ends just there is the one that breaks most often in a fall, or what we call the distal radius.¹ Behind it sit eight small carpal bones, and the one of those that gives way most often is the boat shaped bone under the base of the thumb, or what we call the scaphoid.² Broken and fractured mean the same thing here. What changes the treatment is whether the ends of the bone are still sitting where they belong.

An aligned break and a displaced break are not the same decision

This is what people are really asking. A break that is still in line, or what Dr. Knight calls anatomic alignment, can be held in a cast while it knits. A break where the pieces have moved has to be put back in place first, and in an adult that usually means an operation to hold it there. Underneath that sits a second question, whether the crack runs into the joint surface itself, because the surface the wrist glides on has to be smooth. Here is the test you can apply at your visit, two questions to ask out loud while you are looking at your own x-ray. Ask whether the fracture is displaced, and ask whether it goes into the joint. Those two answers drive everything that follows.

What is done	A fiberglass cast, changed periodically with fresh x-rays to check the bone is holding position. For a scaphoid fracture it usually extends onto the thumb.2	You are asleep for about thirty minutes. The bone is put back in place and held, most often with a plate and screws, sometimes with wires or a compression screw.
How long	Dr. Knight puts a cast at at least six weeks, and longer for the scaphoid, which is slow to heal.2	Set at the visit. Wires placed to hold joint fragments come out later in a short second procedure.
Where	In the office.	At Legent Hospital in Plano, Texas.

Surgery is not where this starts. It is where it goes when the bone will not sit correctly without it.

When to call us

• The wrist looks bent or crooked after a fall.
• The fingers are numb, tingling, pale, or cold.
• Pain and swelling are getting worse rather than settling.
• You were told the wrist was broken and put in a splint, and the pain has changed since.
• The cast is loose, cracked, wet through, or pressing on one spot.
